ACCIDENT/INCIDENT REPORTING AND INVESTIGATION
6.01 INCIDENT REPORTING

Teammates are responsible for reporting all incidents, injuries and workplace
exposures immediately, regardless of the severity.
Failure to report a work-related incident is cause for disciplinary action based
upon our progressive disciplinary policy and guidelines.
Incidents resulting in death, amputation, disfigurement and/or injury requiring
a hospital stay must be reported to Management and the Safety Coordinator.
The Company will contact OSHA as soon as all the relevant facts are
established, but no later than the 8-hour reporting requirement.

The Safety Coordinator will visit the scene of a serious incident as soon as
possible to conduct the incident investigation.
If disciplinary action is recommended, it will be accomplished in compliance with
established disciplinary procedures and in consultation with Management.
Findings and procedures will be documented and records maintained.
6.02 INCIDENT INVESTIGATION
Our policy is to investigate all incidents, injuries, exposures and near misses to better protect our
teammates and to prevent a recurrence. Procedures for investigating workplace incidents,
injuries and exposures are as follows:

The injured Teammate or supervisor will notify the main office immediately.
The reporting supervisor will investigate the incident and complete the
Supervisor’s Incident Report. Any and all equipment needed to properly
complete the investigation will be provided to the reporting supervisor (to
ensure correct completion of the report). Additionally, the supervisor will ensure
the Teammate and witness statements are completed.
At the onset of the investigation initial identification/assessment of evidence is
our top priority; collection, preservation, and security of all gathered evidence
is paramount to successful completion of the investigation. All documentation
must be saved to the TEAMWRKX Shared drive for company record keeping.
The reporting manager will get the Teammate to complete their information if
appropriate at the time the injury occurs unless it is an immediate emergency.
If so, the Teammate information will be obtained later. Interviews and meetings
with witnesses need to be recorded and documented in the Shared drive like all
other pieces of evidence for the investigation.
