A completed "Employer's First Report of Injury" form for each reportable
incident.
Entry made on the OSHA 200 form for each reportable incident. This form will
be posted each year by February 1, and will remain in place until March 1.
The OSHA 300A Summary will be completed and signed by the Chief Safety
Officer. Upon completion, this will be posted in places visible to all teammates.
It will be posted from February 1st of the year through to April 30th of the same
year.
All records will be maintained for 5 years at the TEAMWRKX Construction, Inc.
offices.

The following safety procedures shall be adhered to at all times:
o Check your work area to determine if a problem or hazard may exist.
o Since your activity may endanger other teammates, fellow tradespeople,
or the public, you must take all steps necessary to safeguard all parties
involved or affected.
o Review the safety requirements for each work assignment, as needed,
with your supervisor. You will not be expected or permitted to do your
work in a manner which might result in injury to yourself or anyone else.
All teammates shall acquaint themselves with emergency procedures and
anticipate what you will do in an emergency.
All unsafe or defective equipment, hazardous conditions, and/or unsafe
practices and behavior of other workers and/or the public shall be reported to
your supervisor.
Any Teammate engaged in horseplay or roughhousing may be subject to
immediate discharge.
Teammates shall not be permitted to work if known or reasonably believed to
be under the influence of intoxicating liquor or drugs or are performing in
violation of this IIPP. If found to be under the influence of intoxicating liquor or
drugs they must be removed from the client site/facilities.
Teammates shall not knowingly be permitted or required to work while their
ability or alertness is impaired by fatigue, illness, and/or other causes that there
may be a greater likelihood of exposure to injury.
All work-related injuries and illnesses shall immediately be reported to your
supervisor, or as soon as possible prior to the end of the work shift. Failure to
promptly report any work injury or illness will result in disciplinary action.
