teammate with them. Entrants shall maintain contact with attendants and
supervisors outside as well via walkie-talkie or cellphone. Entrants shall also
have first aid kits and (as we are an earthquake zone) have an earthquake
preparedness kit inside as well to aid survivability in the event of a cave-in.
Attendant = Any person who is appointed or designated to be the attendant at
the entrance/exit of the confined space / permit confined space must be
certified in working in and around said confined spaces. They must keep track of
how many teammates are allowed inside the confined space and cannot exceed
the maximum number established by the supervisor. Attendants must also track
how long the teammates are inside and ensure that they record their
information prior to entering the space. Attendants must have their phone or
walkie-talkie working at all times so that they can remain in contact with the
Entrants and Supervisors. The attendant could very well be the first person to
alert the job in the event of a cave-in, and therefore must have all the emergency
contact info made readily available to them. They play a huge role in the
entrants’ safety.
Entry Supervisor = Any person who is appointed or designated to be the
supervisor of the confined space entry point and operations within the confined
space. This person must be certified and experienced with confined spaces, as
their judgment directly impacts the safety of all the entrants and attendants.
This is the teammate who establishes how many people and which trades may
work within the space. With the proper training and certification, a general
superintendent could be the Entry Supervisor. This individual must have a
walkie-talkie or a cellphone from which they could reach all attendants and
entrants. Furthermore, they are responsible for the condition of the confined
space, as well as anyone entering it. The entry supervisor also oversees the
attendants and can tell people when teammates can and cannot be in the space,
as well as what can and cannot be done within the space.
When two (2) or more companies are occupying the same confined space, the attendant
must be cognizant of the number of entrants, regardless of company. For example, if only
ten (10) workers are allowed in the space, but there are four (4) companies involved and
one of them needs an extra teammate inside to complete work, then it is the duty of the
superintendent and attendant to communicate with the entering parties to confirm who
can and cannot occupy the space. When a teammate leaves the confined space, then the
Confined Space Entry Permit shall be closed and terminated via the entrant’s wet
signature, followed by the attendant’s wet signature.
To ensure that proper protocol is followed, the Safety Officers will conduct an annual
review that will be documented/recorded in the TEAMWRKX Construction Shared Drive.
Furthermore, to protect our teammates entering and around confined spaces, the air
quality will be monitored and reported to the team before entry and during their
occupancy of the confined space. Additional monitoring can be requested at any time by
any teammates associated with working in the confined space. TEAMWRKX Construction
